2008-2009 Back To School Forms

The forms with names in BOLD must be returned by every student/family.

• Sudbury Public Schools Confidential Health and Emergency Information Form (Mandatory, one per family)

• Permanent Dismissal Form (Mandatory, one per student)
• 2008-2009 School Directory Update Form (Mandatory, one per family.)  Please return directly to Kim Cullen, as instructed on the form)  All Families please submit as soon as possible!
• Over-the-Counter Medication Form (Mandatory, one per student)

• Prescription Medication Form (One per student, as applicable)
• Daily Medication Care Plan (One per student, as needed)
• Asthma Forms (One per student, as needed)
• Allergy Forms (One per student, as needed)
• METCO Cooperating Family Form (As applicable)
• School Improvement Council Nomination Form (As applicable)
• 2008-2009 Free and Reduced Price School Meals Form (One per family, as needed) - Available in August
